Health Guarantee /Purchase Agreement:
1. All deposits are NON-REFUNDABLE unless the puppy, which the deposit is held for, should pass while under the care of the breeder. Should the above occur, the buyer may have the choice of either: 1) Selecting another puppy of the same sex of equal value, or 2) Selecting out of the next available litter; the same sex and of equal value. If payment is made in full during this time, the buyer will have the same options as stated above.
2. The puppy is guaranteed to be in good health at the time of sale.
3. The seller must receive a copy of this Health Guarantee signed by the buyer before the puppy will be shipped or turned over to the Buyer. We provide a complete health record with schedule of vaccinations, worming and registration application papers.
4. The buyer has 72 hours to get a veterinarian's exam (at buyer’s cost) after receiving the puppy for the guarantee to be valid. If, in this time frame, the puppy is found to have a fatal genetic disorder, it must be returned to the seller within 24 hours of findings. If there is shipping involved, it will be at buyer’s cost. 100% of the purchase amount will be refunded in a manner deemed timely by the seller. If the veterinarian finds any fatal defects with your puppy, it can be exchanged for another puppy of equal value of the customer's choice immediately, or as soon as one becomes available, provided a letter from the examining veterinarian is offered as evidence of the animal/s illness. The puppy will then be re-examined by my vet of choice as a second opinion.
5. If the puppy should die before reaching 12 months of age (1 year), an autopsy shall be performed to determine the exact cause of death at buyer’s cost. Buyer’s veterinarian shall send results in writing to seller’s veterinarian along with a photograph of the deceased dog.
6.The seller assumes no responsibility for any medical / veterinarian costs incurred by the dog. The seller is not responsible for any freight or other charges incurred in returning or replacing the dog. You, as the owner, are required to: Give all immunizations. Dog is to be checked for internal parasites every 3 months until one year of age. Dog is to be put on heartworm preventative by 4 months of age. (Consult with your veterinarian.) If illness occurs, dog is to receive immediate veterinarian care and all prescribed treatment is to be followed.
7. The seller assumes no responsibility for this animal after leaving the premises for allergies, training difficulties, behavior problems, or any other reasons.
8. If pet was sold with Full Registration, the seller does not guarantee the animal for breeding.
9. At no time will your credit exceed the purchase price of your puppy. We are not responsible for any veterinarian bills you incur with puppies purchased from us.
10. For the safety of the rest of the puppies here at Star Pups Kennel, all sales on puppies are final. Absolutely NO PUPPY will Re-Enter the Kennel.
The guarantee does not include: Kennel cough, Giardia, Bronchitis, Hip Displasia, Respiratory Infection, Crypt orchid, Undershot or Overshot, Show / Breed / Field qualities - lack of, parasites - internal or external, Mites / Ear Mites, Hernias - known or unknown, Hypoglycemia, Colds, Coccida, or any other minor illnesses.
Written proof from a veterinarian will be required to verify all of the above. Buyer understands and agrees that noncompliance of any or all of the terms of this agreement will void any and all obligation that seller has to the buyer.
We have read this contract, understand fully and acknowledge the receipt of same. We are not relying on verbal or written statements not contained herein.

Please return - keep copy for your records. Shots are given according to the requirements of the State of Ohio….any other required shots are responsibility of new owner. If we cannot ship before second and third sets of shots are due, new owner reimburses the cost (if shipping is involved).
